 Consistent
with the established model in Ontario and current best practices,
patients eligible for referral to the clinic will be required
to meet the following criteria:

Body Mass Index (BMI) greater than 40 kg/m2,
or
- BMI greater than 35 kg/m2 with significant obesity-related
co-morbidities
- Acceptable operative risk
- Failure of non-surgical weight loss programs
- Motivated and willing to make eating and lifestyle
changes
- Psychologically stable with realistic expectations
- Supportive family/social environment
- Nicotine free for 3 months prior to initial assessment
and 6 months pre-operatively
- Cancer free for 5 years pre-operatively
- Practising birth control (female patients)
- Absence of active alcohol or substance abuse
- Absence of uncontrolled psychotic or depressive
disorders
